School Year 2007-2008
| SUMMER SCHOOL |
| Students in grades 9-12 enrolled in any credit summer program other than one of-fered by American Heritage/Academy should notify our of• ce and request from the summer school that all paperwork be forwarded to us. |
| COURSE SELECTION SHEET |
| All students enrolled in grades 7 and 8 should have returned their enrichment sheets by this time; all senior high students should have submitted their course choices by this time. Call the Upper School (Heritage or Academy) immediately if this hasn’t been done. |
| IMMUNIZATION |
If you have not submitted a certificate of immunization, please do so immediately. Florida form DH680 will be available at your physician’s office or a county health department and must be completed and signed by a Florida physician. All students must have the original DH680 form on file before they will be allowed to attend school.
If this form has been submitted in the past with all immunization completed, you need not resubmit. Students entering 7th - 12th grades must show evidence of receiving the required hepatitis series of immunizations (3), second measles (MMR preferred), and Td booster. Date of varicella vaccine or year of chickenpox disease is optional. |
| ACTIVITY FEE |
| All Upper School students are required to pay a $275.00 activity fee. An ID card is issued which entitles students to one school yearbook, Spotlight, and admission to all athletic, drama and music events (excluding district competitions) and for computer usage on campus. Pictures will be taken the • rst few school days and cards will be issued a day or two after. ID cards must always be worn. |

| STUDENT PARKING |
| Driving to school and having a car on campus is a privilege reserved for juniors and seniors only. All students who plan to drive must register for the 2007-08 school year during the book sale. The parking fee will be $50.00. No tag will be issued without complete information, and no car may be parked without the tag displayed in the front window. Misuse of the privilege will result in its being lost. All students will park in Lot 1. Due to limited parking, no freshmen or sophomores may have a car on campus. No exceptions! |
| SCHOOL PICTURES |
| The school's new photographer is Weston Photography, 2750 Glades Circle, Weston, FL 33327, Suite 700, phone 954-888-9259. Seniors should have been contacted regarding ap-pointments for their sittings. If a senior has not been contacted he/she should contact Weston Photogra-phy. Individual student pictures for grades 7 through 11 (in uniform) will be taken on September 5, 6, and 7, on campus. |
| COMMUNITY SERVICE REQUIREMENT |
| Effective with the Class of 2008, all senior high students are required to perform a minimum of 120 hours of service during their high school years. Documentation of all service should be obtained when the service is performed, and the documentation should be submitted to the of• ce within 3 months of the service being performed. (Hours that are not submitted in a timely fashion risk being refused.) The Faculty Council who determine who will be inducted into NHS, as well as universities in determining acceptances, prefer to see a pattern of service rather than a block all at once with nothing else. Students should familiarize themselves with the published • yer on what is acceptable and what is not. Seniors with hours totaling 250 hours or more will be recognized at graduation. |
| CELL PHONES |
| Cell phones are allowed on campus. However, any phone which is visible, whether being used or not, during class time will be confiscated and held for parent pick-up. Unfortunately cell phones are now used for cheating and other inappropriate communications. |
| RECYCLE |
| We ask you to drop off old cell phones (with chargers), old eyeglasses, toner cartridges and hotel complimentary toiletries. Any school office will take your donations throughout the year. The Lions Club and Women in Distress have appreciated our donations since 2002. |
| STUDENT INSURANCE |
| A student accident medical policy is provided as part of your enrollment package. Coverage is for injury during the hours and days when school is in session and while attending or participating in school sponsored and supervised activities on or off school premises. Benefits are the maximum amount payable for covered expenses not recoverable from another plan providing medical benefits. This is a secondary coverage plan, but if there is no other coverage, benefits are payable to a maximum of $1,000,000 (any one accident) and the excess coverage provision will not apply. |
