Immunization and Health Requirements
2008 - 2009

All students attending public or private schools in the state of Florida are required to provide the following documentation:

1. FORM DH 3040: Student Health Examination

2. FORM DH 680:

Immunization Record
PK3 & PK4: DPT (4), OPV (4), MMR (1)
K-6th: DPT (5), polio (4), Measles (2), Mumps (1), Rubella (1), Hepatitis (3)
7th-11th: Measles (2), Hepatitis (3), Tetanus booster (Td)

The state requires completion of Hepatitis B Series for grades K-5 and 7-12 for the 2008-2009 school year. Students must have completed at least the first dose to enter school in these grades, and must provide documentation of successive doses as they are received.

Varicella (chicken pox) vaccine is required for entry and attendance in preschool and Kindergarten, with grades one through six included in the 2008/09 school year. Susceptible students 13 years of age or older should receive 2 doses of the vaccine, given at least 4 weeks apart. Varicella vaccine is not required if the child has documentation of a history of varicella disease.

Students transferring from Broward County Schools should have proper documentation included with their transcripts sent from records retention. Please confirm compliance of Hepatitis/Varicella for the above grades as well as second measles and tetanus booster (Td at 11-12 yrs. if 5 years have elapsed since last dose; subsequent boosters recommended every 10 years thereafter.)

Students entering from out of state or out of the country MUST have immunization records transferred to Florida forms (DH 680/3040) by a Florida physician within 30 days. This can be done at the Broward County Health Department(954 327-6008) if students are not yet established with a local physician.
