| Emergency Information Cards (Lower School) |
Complete both sides in full and return to Office/Clinic(peach card/office; cream card/clinic). The cards must be completed by the first day of school. It is imperative that the clinic be informed of all pertinent medical information for each students (i.e. allergies, medications, chronic conditions, etc.) as ell as telephone numbers (home, work, cell, pager) for parents/guardians and THREE additional persons that may be contacted if parents are unavailable. |
 Authorization for Medication form |
NO MEDICATION MAY BE CARRIED/ADMINISTERED BY STUDENTS ON SCHOOL GROUNDS. According to Florida State Law 232.46,an Authorization for Medication form is to be completed for all students receiving prescribed or over-the-counter medication administered by school personnel (Form included in August mailout). To insure proper administration of medication on the first day of school, please return completed form with medication to respective clinic by Friday August 22, 2003. |

They will be provided & must be completed at the Book Sale August 18 - 22 in the BLACK BOX THEATER. Please come prepared with names and phone numbers of alternative contact persons (3) in case of emergency. Emergency cards will not be included in the summer mailout. |
